Our Cardiac Rehab Phase IV sessions are designed to help patients regain their full physical, psychological, and social status. Our ultimate goal is to help improve the quality of life of our patients and promote secondary prevention.
Our Phase IV sessions build on the progress made in Phase III, which takes place in a hospital environment. In Phase IV, our highly trained cardiac rehabilitation team will continue to support patients in the community. Our instructors will provide guidance and reassurance in a safe environment, tailored to the individual goals and needs of each patient.
We offer the opportunity for patients to progress to specialised gym programs, independent gym use, and other centre activities. Our main aim is to encourage maintenance and lifestyle changes, allowing patients to socialize with others who have shared similar experiences.
Who is Cardiac Rehab Phase IV for?
Patients who will benefit from receiving cardiac rehabilitation are those who have:
· Had an acute cardiac event e.g. myocardial infarction
· Had or are awaiting revascularization ( coronary artery bypass graft surgery or
percutaneous coronary intervention
· Stable angina
· Heart failure
· Has valve surgery
· Has heart transplantation
· Had a cardioverter defibrillator implanted

How to access this scheme:
Access to our Phase IV programme is given by your exercise physiologist at the end of your Phase III rehabilitation in the hospital setting. If you have not received this and would like to join the scheme in Corby, please consult the hospital where you are receiving treatment.
